A representative of David Lawrence Center presented their risk management program at the Mental Health Corporation of America (MHCA) Conference in Clearwater, Florida on February 25, 2010 and again at a workshop at the National Council Conference in Orlando, Florida on March 16, 2010. The Negley Awards were established in 1990 by Negley Associates, underwriting managers for the Mental Health Risk Retention Group. The Awards recognize and reward outstanding achievements in risk management by community behavioral health centers, such as David Lawrence Center. UNI/CARE - Pro-Filer™ Reporting: Finding a record that was in effect at the time of service.

Normally when we are writing reports in Pro-Filer™ we want to see the value of a field at some particular time. Usually we are relating to the recorded service. For example, in a recent report request, I was asked to provide a list of services with the client’s GAF (Axis5) that was effective at the time of service.

How do we go about this? First of all, I’m not going to give you the entire query or Crystal Report, just the bits of code (Snippets) that define our service and GAF score. I keep a text file with a bunch of these snippets that I can refer to and just copy and paste code as needed.

So, let’s grab our recorded service. The start of the service is the STARTTIME field in the RECORDED_SERVICE table.

     RECORDED_SERVICE.STARTTIME

Now, let’s make sure that our GAF score was EFFECTIVE on or before the RECORDED_SERVICE.STARTTIME. This information is stored in the ClientAxis5 table NOT the Axis5 table. So our query starts like this:

     WHERE ClientAxis5.EffDate <= RECORDED_SERVICE.STARTTIME

This is only part of the solution however, now we have to filter out all those records that were expired previous to the recorded service. So we examine the ClientAxis5 expiration date for that. Since there may be new GAF scores that were entered after our recorded service, we have to find the one that was not expired at the time. This is how our query will look:

     WHERE ClientAxis5.EffDate <= RECORDED_SERVICE.STARTTIME
     AND (ClientAxis5.ExpDate IS NULL OR ClientAxis5.ExpDate >=
     RECORDED_SERVICE.STARTTIME)

Pay close attention to the parentheses around the second line. They are critical. So now we have a query that is first checking to be sure the GAF was entered into the system prior to the service and we are checking to be sure that the expiration date is null or was expired after our recorded service.

This query can be used throughout the Pro-Filer™ database for various other fields as well. Cost Center Locations, Client Demographics and just about anything else you would want to know.

UNI/CARE - Pro-Filer™ News: New FREE Data Warehouse Solution

UNI/CARE and ITX Enterprises are giving you a solution to the number one issue that face most agencies, that is the difficulty of reporting against the Pro-Filer™ database.  The Pro-Filer™ database has been designed to do what it does very efficiently and as a production database it functions very well.  It is, however rather difficult to report from.  This is really no longer an issue with this announcement from UNI/CARE and ITX Enterprises.

It is never a good idea to report from any production or transaction database on any system, not just Pro-Filer™.  Frankly I have often said "If you are reporting from your production database, then you are doing it wrong."  Reporting from any large database should always be done from a data warehouse.   Today with the complexity of all EHR databases rising to meet the demands placed on them, it has become time to seriously consider implementing a data warehouse solution within your organization.  

There are a number of very strong benefits to adding a data warehouse to your Pro-Filer™ system.  First, it removes the reporting load from your production server, I can tell you that after 8 years experience on Pro-Filer™; the number one cause for slow-downs, locking and blocking is reports.  Secondly, your reports will run faster, they are no longer sharing resources with the Pro-Filer™ program or needing access to the same records concurrently.

I conceived of this data warehouse solution while with ITX Enterprises in order to solve the same issues for the HDS Utah Consortium that you are seeing with your Pro-Filer™ implementation today.  It is a strong testament to UNI/CARE's continuing product improvement initiatives and customer service that they not only offer this solution to all UNI/CARE clients, but do so for free.

At the core of this data warehouse is a series of denormalized datamarts that I authored while at ITX Enterprises which will rapidly improve report writing efficiency and the speed of report processing from Pro-Filer™.  We found that about 90% of all reports; especially QA reports can be run from the datamarts.  When reports have been written and optimized to use these denormalized tables, speed increases of several orders of magnitude have been seen.
